Once accepted, you will receive registration forms to complete prior to your first visit.

You will be invited to join AVA connect where you can book appointments and message the team directly.

Forms can be completed online or in clinic. Please bring a valid Alberta Health Care card to all appointments. Additional ID may be requested if needed.
